Practitioner Training: Dave Hamilton is a licensed Massage & Bodywork Therapist in the state of North Carolina. I have done over a decade in continuing education studies in Somatic Education Polarity Therapy, Ashiatsu, Myofascial Massage, and Trauma Healing. I am certified as a Somatic Experiencing Practitioner by Somatic Experiencing International. I completed the BASE (Bodywork and Somatic Education) training with SEI faculty Dave Berger.
Mission and Scope of Practice: Based on your goals and my observations, I will perform treatment, make assessments and give recommendations to support the healing and learning process. I am neither a physician or a psychologist and will not diagnose or prescribe. I regard the information you share with me with the greatest respect. In order to protect our safety, I adhere to the standards of professional conduct in section .0500 of the NCBMBT Practice Act.
Payment
60 minutes / $80
90 minutes / $100
Cancellation
Please contact me at least 24 hours in advance to change or cancel an existing appointment. With the exception of personal or family emergency, clients will be charged the full amount for a session canceled or changed with less than 24 hours notice.
Termination of Session
It is your responsibility to tell me if you are uncomfortable with any aspect of the sessions and to inform me of any changes to your health or medical status. You as the client and I as the practitioner have a right to refuse touch or to refuse or terminate treatment at any time.
Somatic Experiencing Consent
I understand that Somatic Experiencing employs awareness of bodily sensation, images, movements, emotional expression and thought to help heal rather than reenact or relive traumatic events.
Full disclosure
I affirm that I have stated all known medical conditions and answered all questions honestly. I agree to keep the practitioner updated to any changes in my medical profile and understand that there shall be no liability on the practitioner’s part should I fail to do so. I also understand that any illicit or sexually suggestive remarks or advances by me will result in the immediate termination of the session, and I will be liable for payment of the scheduled appointment.
